Short-term disability policy       Long-term disability policy

The Genesee County disability insurance carrier is The Lincoln Financial Group.  This employer-paid benefit is available after the completion of 520 straight-time hours of employment.  Short-term disability coverage is available for a maximum of 26 weeks at 60% of the employee's gross weekly salary up to a set maximum amount which varies among bargaining units.  Long-term disability coverage could be available for up to ten (10) years or up to age sixty-five (65), whichever occurs first.  For employees with less than 5 years of service, short and long-term disability is available on a time for time basis.  Coverage amounts vary as follows:

Disability Income Application Information

Determination of eligibility and payment of benefits is a function of the insurance carrier.  Employees are bound by the terms and conditions of the carrier. Employees may pick-up the necessary forms from the Human Resources Department. 

To download the forms online, please notify the benefits office with your name, phone number, department, and supervisor's name, then you may print out the Lincoln Disability Claim Form with Certification of Health Care Provider. Please review the instructions. You may want to have your disability income directly deposited to your bank account. Otherwise, payments will be mailed to your home address on a weekly basis.

 

The obtaining or receiving of disability leave claim forms from the Human Resources Department does not fulfill the employee's contractual obligation to comply with the reporting of absences requirements.
